how do you add or rearrange the settings and logout text in the top right corner and maybe add more links there?

That's not configurable, you'll need to modify the source code to do that, check js/common/webmail.js. Examining the web application layout with Firebug extension of Mozilla Firebug or with similar tool in other browser will get you a decent starting point as to elements you need to look for.
