If the user prefer to leave a copy of messages on the server, how to know if the messages have been downloaded?

This depends on whether POP3 or IMAP protocol is used. The only way to achieve this for POP3 is to maintain a database which will contain UIDs of downloaded messages. The following Knowledge Base article may be helpful in this regard:

With regard to IMAP, it supports read/unread message status, so you can search for read/unread messages.
